Dental Claim Form
V1.09 Software Operating Instructions
SOFTWARE
INSTALLATION:
installing the software is easy starting from a download link and one click.
The installation program will start and on the installation menu choose NEXT
then NEXT again, and then FINISH.
The Main Menu for the blank SmartForms Dental Claim Form
will appear on your computer ready to use and an icon for the CMS-1500 program
will be added to your desktop for future use.
It is recommended that you review the six menu items found
along the top of the main menu page; File, Edit, Field, MENU,
Text File and Template. You
will need to perform two, one time
tasks before starting; 1) preparing your template and 2) aligning the red
pre-printed CMS-1500 forms with your printer.
If you do not plan on using red preprinted forms, skip the alignment
process.
TEMPLATE: The template function allows the information that is
included on all your claims to be pre-filled on your claim forms prior to
entering specific patient information. On
the Main Menu page, one of the menu items at top is “Template.” Click on Template and click New.
Fill in all the information that is
common to all your claims. Now click File
and Save As and name your template.
For all future claims, open this file and the information that is
included on all your claims will already be pre-filled. You may create more than one template if
needed by giving each template a unique name.
For example if you are billing for multiple doctors, you can save a
template for each doctor or service provider.
PRINTING: SmartForms Dental Claim Form will print claims
on plain paper, generating the form and content. To print a claim, click the
top left menu item File, and Print Menu and choose Print
on Plain Paper. The claim will
appear in a print preview and next you will click the Printer icon
and Close.
ELECTRONIC SUBMISSIONS: Engage a clearinghouse to process your electronic
claim submissions. You will save your
files two ways if you are using a clearinghouse, once as a claim form (.frm) and a second time as a text file (.txt) Follow the
Saving Claims instructions above and next click on the top menu item TextFile and give the file a name. The clearinghouse will access your directory
and take the text files for processing.
